For the past few months I’m noticing that Google is sending some traffic to my website where there are increasing number of queries regarding mobile phone SIM Card re-issue or replacement and people want to know the sample letter format for this purpose. So my post here today is about SIM Card Re-issue or Replacement.
Look my dear friends, normally you’ll not need to write a letter to your mobile operator for SIM Card re-issue or replacement in cases of loss or theft etc. If you are the owner of the SIM Card, if the connection is in your name, then just go and visit your operator’s store or mini store or customer relationship center in your city.
There is a specific form available with every operator which is for SIM Card re-issue, or SIM Card replacement or Duplicate SIM Card issuance.
Just fill up that form and remember to take a photocopy of identity proof, whichever you submitted while obtaining the connection and also a photograph. That’s it. Nothing else will be required. In case of theft, some operators you may require you to submit a copy of General Diary filed in your local police station.
You will need to write a letter only and only if you wish to discontinue or deactivate your mobile connection (or if a SIM is stolen or lost), whether temporarily or permanently. In that case also, you’ll need to submit a proof of identity (but not a photo). In that case, just address the letter to <Your Mobile Operator’s name>, <Address or just city name> and then body of letter is simply I lost my SIM card number (write that long sequence of digits printed on back of SIM packet you purchased) / Mobile number (if you don’t have the SIM packet) etc. and request to re-issue a fresh SIM card to you. That’s all.
SIM Re-issue or Replacement may not be free, operators normally charge a few bucks for that.
